Healthy Hearts Programme: Countryside walks help to cut hospital re–admissions
Cardiac patients are joining sedentary people who wish to become more active in a programme that is cutting hospital readmissions and improving quality of life. The Healthy Hearts Programme run by Caerphilly Cardiac Rehab scheme in partnership with the countryside service of Caerphilly County Borough Council is a GP referral programme that targets residents of Caerphilly who have a cardiac history, and those who have a greater than 20 per cent cardiovascular disease risk, which includes diabetes patients. The programme consists of exercise and education sessions but also countryside led walks. Although the scheme specifically targets cardiac patients, sedentary people are also welcome on the walks, so that anyone can attend and improve their health. According to the findings from last year, all patients who completed the programme avoided readmission to hospital for cardiac reasons. Marked improvements were also seen in patient’s fitness, quality of life, anxiety and depression levels. For further information visit website: www.wales.nhs.uk.
